HYGÉE+ is the first “independent health” platform for chronic disease management and virtual monitoring.

The platform collects and streamlines clinical data from any source and includes remote physiological monitoring, laboratory analysis, electronic medical records, imaging and patient-generated observations. It streamlines Clean Data input data for AI applications and clinical decision support tools.

Finally, HYGÉE+ provides an ongoing medical patient overview that sends notifications and alerts to the care team and patient to support adherence to care plans and predict decompensation. All of these features help automate the delivery of safe, fast, efficient and patient-centred chronic disease management.

HYGÉE+ is accessible via smartphone, desktop or tablet, and allows:

  • Capture and track patient health data.
  • Capture and track patient wellness data (steps, distance travelled, sleep hours, energy expenditure, etc.)
  • Access the history of the client’s results in table or graph format and forward to their referring physician as required.
  • Notify the nurses if there is an anomaly in the values entered by the patient.
